Medical Insurance Plan
Who Pays: You & Schuyler Hospital
When you are Eligible:
Regular full-time and regular part-time employees continuously scheduled to work at least 16 hours per week are eligible the first day of the month following date of hire.

Dental Insurance Plan
Who Pays: You & Schuyler Hospital
When you are Eligible:
Regular full-time and regular part-time employees continuously scheduled to work at least 16 hours per week are eligible the first day of the month following employment.

401(k) Retirement Plan 
Who Pays: Schuyler Hospital
When you are Eligible:
Employees over age 21, having completed one continuous year of employment with Schuyler Hospital, are automatically enrolled in the 401(k) Plan. Contributions are made by the Hospital, there is no match or contribution required by the Employee. Vesting occurs after five qualifying years. Employees who are not yet eligible for the Hospital benefit may enroll and contribute their own contributions at any time.  Employee contributions are considered to be fully vested immediately.

Life Insurance
Who Pays: Schuyler Hospital
When you are Eligible:
Regular full-time and regular part-time hourly employees continuously scheduled to work at least 16 hours per week are eligible the first day of the month following employment.
Regular full-time and regular part-time salaried employees continuously scheduled to work at least 16 hours per week are eligible the first day of employment.
What you Receive:
Hourly employees are eligible for $10,000.
Salaried employees are eligible for twice their annual salary.  See Summary Plan Description for the Life Insurance Plan.

Vision
Who Pays: You
When you are Eligible:
Regular full-time and regular part-time employees continuously scheduled to work at least 16 hours per week are eligible the first day of the month following employment.
What you Receive:

Short Term Disability
Who Pays: You & Schuyler Hospital
When you are Eligible:
After four weeks of continuous employment.  All employees.
What you Receive:
Provides 50% of lost wages to a maximum of $170 per week following a one-week waiting period.

Social Security
Who Pays: You & Schuyler Hospital
When you are Eligible:
Upon employment. All employees.
What you Receive:
Provides retirement and disability benefits for you and your eligible dependent(s).

403(b) Savings Plan
Who Pays: You
When you are Eligible:
Employees that are at least 18 and continuously scheduled to work at least 16 hours per week are eligible to set aside tax-deferred savings toward retirement.
What you Receive:
Information packets are available through Human Resources.

Employee Assistance Program (EAP)
Who Pays: Schuyler Hospital
When you are Eligible:
Upon employment. All employees.
What you Receive:
Information, assessment, short-term counseling, and/or referral for personal/professional problems.

Extended Illness
Who Pays: Schuyler Hospital
When you are Eligible:
Regular full-time and regular part-time employees continuously scheduled to work at least 16 hours per week are eligible for Extended Illness the pay period following the completion of 90 days of employment.  Supervisory approval required.
What you Receive:
Extended Illness accrues based on worked hours.  See Human Resource Policy and Procedure Book.

Bereavement Leave
Who Pays: Schuyler Hospital
When you are Eligible:
Upon employment. Regular full-time and regular part-time employees continuously scheduled to work at least 16 hours per week.
What you Receive:
Up to 3 consecutive days for the death of immediate family members. See “Death in Family” policy in the Human Resource Policy & Procedure Book.
 

Jury Duty
Who Pays: Schuyler Hospital
When you are Eligible:
Upon employment. Regular full-time and regular part-time employees continuously scheduled to work at least 16 hours per week. All employees.
What you Receive:
Schuyler Hospital will pay your regular pay for scheduled workdays.

Finger Lakes Federal Credit Union
Who Pays: N/A
When you are Eligible:
Upon employment. All employees.
What you Receive:
Allows for all banking operations on site.

Service Awards
Who Pays: Schuyler Hospital
When you are Eligible:
Every 5 years of continuous services.  All employees except temporaries.
What you Receive:
Recognition of service.

Paid Benefit Time  (PBT)
Who Pays: Schuyler Hospital
When you are Eligible:
Regular full-time and regular part-time employees continuously scheduled to work at least 16 hours per week are eligible for Paid Benefit Time the pay period following the completion of 90 days of employment.  Supervisory approval required.
What you Receive:
Paid Benefit Time accrues based on worked hours.  Cash back provisions available after one year of service.  See Human Resource Policy and Procedure Book.

Flexible Spending
Who Pays: You
When you are Eligible:
Upon satisfactory completion of the 90-day introductory period of employment, regular full- and part-time employees regularly scheduled to work at least 16 hours per week are eligible to enroll in the Flexible Spending Accounts.
What you Receive:
Employees can reduce taxable income by deferring monies into an account to reimburse for eligible out-of-pocket medical and/or dependent care expenses.
