ADMISSION INFORMATION

On the day of your scheduled admission, go to the admissions area near the emergency room. While you are in the admissions area, you will be asked to present your insurance cards, provide billing information, and sign some forms. You may be directed to the lab, x-ray, or cardiopulmonary departments for initial testing if ordered by your physician.
 
Your admitting interview will be processed efficiently and effectively, after which you will continue to your assigned room. We will make every effort to provide you with the room accommodations of your choice, provided it is consistent with your medical care. Private rooms, when available, are an additional rate.
 
IDENTIFICATION BRACELET
The wrist identification bracelet that you receive upon admission should not be removed during your stay. It provides positive identification to all those who serve you and acts as a safeguard for your protection.
 
PREADMISSION TESTING
The purpose of preadmission testing is to shorten your hospital stay. Tests ordered by your physician are performed on an outpatient basis in advance of your surgery.
 
CONSENTS
Consent forms for treatment or operations must be signed by each patient. Patients under 18 years of age must have a parent or legal guardian sign for them. If you do not understand one of the forms, please ask for an explanation.
 
WHAT TO BRING
Please bring only a few personal things you will need - toilet articles, pajamas, robe, slippers, reading material, and small amounts of money for newspapers.

Maternity patients should bring clothes for their baby to wear home from the hospital.
 
Dentures, eyeglasses, or contact lenses should be placed in protective containers when not in use. The hospital cannot be responsible for breakage or loss of such articles.
 
Please remember to bring all insurance cards and information that apply to your situation. Be sure you have the name, address, and telephone number of your next of kin and at least one other person who you would like to have notified in case of an emergency.
 
WHAT NOT TO BRING
Please leave valuables at home or send them home with members of your family. Although the hospital cannot be responsible for money or other valuables kept in your room, valuables can be locked in a safe in the admissions area.
 
MEDICATIONS FROM HOME
Your doctor will prescribe the drugs that you need during your hospital stay. Unless your doctor asks you to bring medications, please leave them at home.
